Ready-to-Wear Re-Orders
If
you are an existing CMS patient and would like to re-order your
ready-to-wear garments, please email us by clicking
here. Please include the following in your email:

Please let us
know if this will be an exact re-order, or if there are
changes from your previous order.

If there are
changes, please include the changes in your email. Example:
I would like 2 pair of garments in black instead of beige.

Include how you
would like to receive your garments. You can choose to come
to our office for pick-up, or you can have the garments
shipped to you. Please note that the shipping fee is
$15.00.

Let us know how
your prescription will be sent to our office. You can
contact your physician’s office and have them fax a
prescription directly to us, or you can mail it to the
office in which you are seen.
NOTE: The Rx and
shipping fee (if you choose shipping) need to be received
before we can distribute products.
If you are not a
patient at CMS, but would like to be measured for garments,
please call 877.CMS.4415 to schedule an appointment for an
assessment. |
